New Patient Process
The first visit will allow you and the doctor to meet and better understand the availability of the highest quality of care on the road to recovery. The doctor will discuss your history, your condition, the
examination, treatment options, and more importantly, the goals of the patient.
We understand that time and convenience is vital. Therefore, to provide efficient and effective treatment, our facility provides a complete range of healthcare services, including Chiropractic Care, Nutritional Consults, Physical Therapy, as well as other Rehabilitative services.
Prior to the Visit
Before arriving at Optimal Healing, please call the office to set up your appointment. During the call, you will be asked for your name, contact phone number, and your general symptoms.
New Patient Paperwork
To shorten the time of the visit, please print and fill out our New Patient Paperwork prior to your visit. The new patient paperwork provides the doctor with your general information. During your visit, you may be asked to fill out additional paperwork depending on the area of concern. Please bring the completed new patient paperwork to your first appointment.
We will gladly confirm your coverage with your insurance company and try our best to help set up an affordable treatment plan. If you would like to check your own benefits prior to the appointment, please refer to the Insurance Verification Form as you will be responsible for any additional costs not covered by the insurance company.
We accept payment in the form of Visa, Mastercard, Discover, American Express, Check, or Cash. Payment is due at the time of services rendered. Please refer to the Insurance Page for further information. Also, we offer payment plan options, which ensure that you continue to receive treatment and services at an affordable price. This can be discussed further during your visit.
Items to bring to the first visit:

- ∙ Any diagnostic imaging (if pertaining to cause of the visit)
- ∙ Appropriate clothing (for area of examination or possible for physical therapy)
On your first visit
On your first visit, please allow approximately 60-90 minutes for the initial examination evaluation. You will be given new patient paperwork prior to seeing the doctor (if you have not already completed it). The New Patient Paperwork can be downloaded, printed, and filled out before arriving to the visit. If the new patient paperwork cannot be completed prior to the visit, please arrive at least 20 minutes prior to the appointment to allow enough time to complete the paperwork.
Upon meeting with Dr. Mina Patel, you will discuss the concerns and expectations of your care. Together, you and the doctor will determine the best treatment option from the various services offered at the facility. A prompt referral can be advised if your condition would be better treated, or co-managed by another healthcare provider.
History & Examination
Next, the doctor will gather a detailed medical history, which will help the doctor perform a thorough neurological, orthopedic, and physical examination. In addition, specialized functional testing, such as gait analysis, postural analysis, and joint and muscle testing, may be performed to determine an accurate diagnosis. Dr. Patel will explain the details of each examination, along with the reason for its requirement.
The doctor will provide the information and answers to the questions pertaining to your condition. After the educating you on your treatment plan and having gained knowledge of the condition, the doctor, together with your views and compliance, will determine the most effective therapy for your needs.
Prior to leaving the office, specific home instructions may be advised. These include ice/heat applications, avoidance of certain movements, activities, or positions, along with exercises.
For your subsequent visits, allow approximately 30-60 minutes depending on the treatment plan. Each visit may be different than the previous one, and will depend on the progression of your recovery.
If you are unable to keep an appointment, we ask you kindly to provide us with at least 24 hours’ notice for the cancellation. During the time of the cancellation, we can find a better time that is convenient for your busy schedule.
